AUSTRALIAN FOOTBALL
All AFL club medical officers conduct baseline neuropsychological testing pre-season, which is used as a guide for assessment and treatment of players who have suffered concussion.
There are no mandatory requirements for exclusion of players for a pre-determined time following a head injury or other testing.
Any player who misses a game through concussion must be recorded as part of the AFL's wide-ranging injury survey. But this survey does not account for head injuries where the player returns to play during the game or the following week.
RUGBY LEAGUE
NRL club medical officers are responsible for the assessment and treatment of players who have suffered concussion. This includes on-going investigation, neuropsychological referral and return to training or play.
There are no mandatory requirements for exclusion of players for a pre-determined time following a head injury or other testing, although recommended NRL guidelines for the assessment and handling of these injuries have been issued to all clubs. The NRL medical officers handbook warns treating staff to guard against players and coaching staff applying undue pressure which influences assessment of any concussive episode.
RUGBY UNION
Australian Rugby Union's handling of concussion among players is bound by rules set by the International Rugby Board. The international rules stipulates all junior players who suffer concussion are required to take a three-week break.
Senior players also must take three weeks off. However, if a team physician and neurological specialist gives the green light they can return earlier. No players are allowed to return to the field of play if knocked out during a game.
SOCCER
There is no mandated international concussion policy used in Australia. Professor Jiri Dvorak, chief medical officer for FIFA, is actively involved in drawing up international guidelines for treatment of concussion.
These guidelines are only recommendations. In the A-League, the incidence of reported concussion is much smaller than the other contact codes.
AMERICAN FOOTBALL
All NFL players must have mandatory baseline neuropsychological testing, a computer program/game to test memory, reaction time, and other cognitive measurements. If a player sustains a loss of consciousness, he should not return to the same game or practice.
For players removed from games due to concussions, repeat testing will be done to track recovery and to help decide when they can return to play. They will be re-tested against their baseline performance the next season at training camp.
A hotline to report a NFL player being forced to practise or play against medical advice was set up last year.

10.1 Concussion

10.1.1 A Player who has suffered concussion shall not participate in any Match
or training session for a minimum period of three weeks from the time of
injury, and may then only do so when symptom free and declared fit after
proper medical examination. Such declaration must be recorded in a
written report prepared by the person who carried out the medical
examination of the Player.
10.1.2 Subject to sub-clause 10.1.3 below, the three week period may be reduced
only if the Player is symptom free and declared fit to play after appropriate
assessment by a properly qualified and recognised neurological specialist.

I'd contend there's no need for a certificate, based upon the initial diagnosis of chronic softness complicated by a mild case of wankerosis
